Testicular Torsion and Male Fertility: Links You Need to Know

Testicular torsion is a medical emergency that occurs when the spermatic cord, which provides blood flow to the testicle, twists and cuts off the blood supply. This condition demands immediate attention due to the risk of permanent damage to the testicle and subsequent complications, including infertility. Testicular torsion primarily affects males in their teenage years but can occur at any age. The urgency of this condition cannot be overstated, as the window for effective treatment to preserve testicular function and fertility is extremely narrow.

The potential impact of testicular torsion on male fertility is significant. Damage to the testicle can result in impaired sperm production, reduced testosterone levels, and long-term fertility issues. Understanding how testicular torsion affects fertility and the importance of prompt medical intervention is crucial for preserving reproductive health.

How Testicular Torsion Affects Fertility

Testicular torsion occurs when the spermatic cord twists, cutting off blood flow to the testicle. The testicles require a constant supply of oxygen-rich blood to function properly, and this blood flow is critical for the survival of the cells responsible for producing sperm and testosterone. When blood flow is interrupted, it can lead to ischemia and subsequent testicular damage.

The primary consequence of testicular torsion is the interruption of blood flow, which deprives the testicle of oxygen and essential nutrients. The Leydig cells in the testicles are responsible for producing testosterone, the hormone critical for male reproductive health and secondary sexual characteristics. Testicular torsion can damage these cells, leading to a decrease in testosterone levels. Low testosterone can affect libido, erectile function, and overall fertility. Chronic low testosterone levels can also have broader health implications, including reduced muscle mass, increased body fat, and decreased bone density.

Research has shown that testicular torsion can have a profound impact on sperm quality, affecting motility, morphology, and count. Studies examining semen samples from men who have experienced testicular torsion reveal a significant decrease in sperm quality compared to those without such a history.

Sperm motility refers to the ability of sperm to move effectively towards an egg. Poor motility can hinder the sperm‘s ability to reach and fertilize the egg, reducing the likelihood of conception. Testicular torsion can lead to a substantial reduction in the percentage of motile sperm, making natural conception more challenging.

Sperm Morphology

Normal sperm morphology is essential for the sperm to penetrate and fertilize the egg. Abnormalities in sperm shape can impair this function. Testicular torsion can increase the incidence of sperm with abnormal morphology, which may affect the overall fertility potential of affected individuals.

Sperm Count

Sperm count is another critical factor in male fertility. A low sperm count can significantly reduce the chances of successful fertilization. Studies have shown that men who have experienced testicular torsion often have lower sperm counts compared to those who have not, further contributing to fertility challenges.

Subfertility
